Nursing Home & Long-Term Care Psychiatry
AgeWell Psychiatry provides specialized psychiatric care for residents of skilled nursing facilities, assisted living communities, rehabilitation centers, memory care settings, and other long-term care environments. Services may include comprehensive psychiatric evaluations, medication management, behavioral health assessments, and ongoing psychiatric follow-up. We work with facility care teams to address residents' changing psychiatric, behavioral, cognitive, and emotional needs while supporting dignity, safety, and quality of life.
Facility-Based Psychiatric Services
AgeWell Psychiatry provides on-site and/or secure telepsychiatry services designed to support residents with psychiatric and behavioral health needs. We collaborate with physicians, nursing staff, social workers, therapists, caregivers, and families when appropriate to promote coordinated and individualized care. Our services are designed to complement the facility's existing care team and support continuity of psychiatric treatment.
Behavioral & Mood Assessment
AgeWell Psychiatry provides comprehensive assessment of changes in mood, behavior, cognition, sleep, and emotional functioning. Evaluations may consider psychiatric and medical history, current medications, treatment response, behavioral changes, and other contributing factors. Findings are used to develop appropriate treatment recommendations and support ongoing psychiatric care tailored to the resident's individual needs.
