Collaborative Facility Care

AgeWell Psychiatry partners with skilled nursing, assisted living, rehabilitation, and long-term care organizations to provide coordinated psychiatric care for residents. We collaborate with medical providers, nursing staff, social services, therapists, and families to support comprehensive assessment, medication management, behavioral health needs, and ongoing psychiatric follow-up. Our goal is to complement the facility's existing care team while promoting resident safety, dignity, stability, and quality of life.
